Patient Activity Report from Department of Justice
All licensed health care practitioners (who are authorized to obtain controlled substances security forms) are able to obtain a "patient" history or activity report from the Department of Justice to assist in identifying those patients who may be "doctor shopping, In order to obtain access to the CURES/PDMP system, physicians must first register with CURES/PDMP by submitting an application form electronically. In addition, the registration must be followed up with a signed copy of the application and notarized copies of the validating documentation which includes: Drug Enforcement Administration Registration, State Medical License and a government issued identification. The application and notarized documents should be mailed to:
