Postgraduate Training Program Resources
To complete the licensing application process, the Board must receive all necessary forms. Reference the Third-party services webpage for electronic signatures accepted by the Board.
Use the tables below to identify the forms needed. All postgraduate and medical school forms must be submitted by the postgraduate training program or medical school to the Board's Direct Online Certification Submission (DOCS) portal. Applicant forms must be submitted by the applicant through BreEZe.

Form EF — Verifies enrollment in an ACGME-accredited postgraduate training program in California.
- You can sign this form before training begins.
Form PTA/PTB — Required to verify participation in all ACGME/RCPSC/CFPC-accredited postgraduate training programs attended. This applies regardless of whether the resident completed the program or received partial or no credit.
- The credit box must list the number of months of credit the resident received at the time the form was signed.
- Sign this form no more than 30 days before the resident completes the training required to qualify for a license (12 months for U.S./Canadian medical graduates or 24 months for international medical graduates).
- Must be signed by the current program director or designated institutional official
(DIO) listed on the ACGME/RCPSC/CFPC website.
- If the ACGME website has not been updated to reflect the current program director information, the current program director may still sign and submit the form to the Board. A letter on official letterhead, signed and dated by the current program director, confirming their active role and indicating that the website has not yet been updated, must be submitted with the PTA/B form.
- If the program director or the DIO is delegating signature authority to another person, evidence of that delegation, on official letterhead and dated within the last 12 months, must be attached to this form.
CTV — Verifies current enrollment in a Board-approved training program to qualify for the Reduced Initial License Fee. Must be currently enrolled in an ACGME/RCPSC/CFPC-accredited postgraduate training program to qualify.
- Must be signed by the current program director or DIO listed on the ACGME/RCPSC/CFPC
website.
- If the ACGME website has not been updated to reflect the current program director’s information, the current program director may still sign and submit the form to the Board. A letter on official letterhead, signed and dated by the current program director, confirming their active role, and indicating that the website has not been updated, must be submitted with the CTV form.
- If the program director or the DIO is delegating signature authority to another person, evidence of that delegation, on official letterhead and dated within the last 12 months, must be attached to this form.
CODA1-CODA2 — Required to verify participation in a postgraduate training program accredited by the Commission on Dental Accreditation.

Form MED — Required from each medical school the applicant attended.
- The form must be signed and dated after the diploma is issued to be accepted.
- If the school marked "yes" to any Unusual Circumstances questions, they must include a signed and dated letter of explanation on medical school letterhead from the current medical school official.
Diploma — Certified copy of medical school diploma.
- For international medical school graduates only.
- The certified copy must have the original signature of the dean or registrar of the medical school affixed with the official medical school seal and include a statement attesting that the copy is a true and correct copy of the original.
- A diploma uploaded to the DOCS portal does not require a medical school seal.
Transcript
- An official medical school transcript and translation (if not in English) is required from each medical school attended.
- Must be prepared on university letterhead affixed with the signature of the dean or registrar, documenting all the basic science and clinical courses completed during the medical curriculum.
- Must have the medical school seal affixed unless the transcript is submitted through DOCS.

Notification of Name Change — Required for any licensee/registrant who changed their legal name after applying.
- Your P&S license must reflect your legal name.
- A current government-issued photographic identification card (e.g., driver license, alien registration, passport, etc.) AND one of the following legal documents as proof of name change:
- Certified Court Order;
- Marriage Certificate, or
- Dissolution of Marriage (Divorce)
- Original documents are not required, photocopies will be accepted. If original documents are mailed, they will be returned by certified mail.
Form EXP — Used to explain, in detail, a "Yes" answer on the license application. Each "Yes" answer requires a separate Form EXP.
Form TOA — A written chronological description of all professional and non-professional activities.
- Physician and Surgeon applicants provide a timeline of activities (TOA) from medical school graduation to current date.
- For Transition applicants, a TOA is needed if there is a gap in training since the date the PTL was issued.
- For PTL applicants, a TOA is needed if there is a gap of more than three (3) months between the graduation date and the start of postgraduate training.

Form ILR — Required for initial license renewals to verify credit obtained or enrollment in a Board-approved postgraduate training program.
PSU1-PSU2 — Required from the program director or designated institutional official within 30 days of the following actions for residents that hold a Postgraduate Training License or Physician's and Surgeon's License: the resident transfers to another program, is terminated, resigns, takes a leave of absence, or has any other program change affecting the license expiration date or status.
